How to File a Claim

Step 1 - Find the plan name
The first step to filing a claim is to locate the name of your plan. This information can be found on your confirmation of coverage or certificate emailed to you at the time of purchase and is necessary to ensure you file a claim with the correct claim form.

Step 2 - Gather the necessary documents
Documentation will vary depending on the type of claim you are filing. Below are some required documents for common claim types. You should always review the claim form for any required documentation. If you have traveled internationally, you may also be asked to provide a copy of your passport showing the identification page and entry/exit stamps or provide a copy of your I-94.

You may need some or all of the documents listed to file your claim
MEDICAL CLAIM TRIP CANCELLATION or TRIP INTERRUPTION TRAVEL DELAY (QUARANTINE)
  • Detailed bills and/or forms for medical services received
  • Receipts for payments made for medical treatment
  • Medical documentation
  • Doctor and/or hospital reports
  • Original Trip Itinerary and Invoice
  • New Trip Itinerary
  • Proof of Trip Payment
  • Proof showing the cancellation date, the amount forfeited, and the amount refunded
  • A statement from Physician; Government; Airline Carrier; or Airport Facility that concerns your Delay.

Step 3 - Complete the claim form
Complete the claim form in its entirety. You will be asked what happened. Please provide a concise summary and be sure to include all pertinent information.

  • What happened
  • When it happened
  • Where the incident occurred
  • Who was involved
  • How were you affected

Step 4 - Submit the claim
Depending on the plan you purchased, and when you purchased it, you may be able to submit your claim online. Otherwise, you can mail or email the claim and all corresponding documents. Please see the Claim Forms page for more info on how to submit your claim.

Important! Please be aware of the following when filing your claim:

  • The claims team may reach out to you via email if additional information is needed to process the claim.
  • Processing your claim will be delayed if you use the wrong claim form, mail the claim to the wrong address, or send in incomplete documentation pertaining to your claim.
  • Any cancellation or flight delay must be documented by the airline.
