Step 1 - Find the plan name
The first step to filing a claim is to locate the name of your plan. This information can be found on your confirmation of coverage or certificate emailed to you at the time of purchase and is necessary to ensure you file a claim with the correct claim form.
Step 2 - Gather the necessary documents
Documentation will vary depending on the type of claim you are filing. Below are some required documents for common claim types. You should always review the claim form for any required documentation. If you have traveled internationally, you may also be asked to provide a copy of your passport showing the identification page and entry/exit stamps.

Step 3 - Complete the claim form
Complete the claim form in its entirety. You will be asked for a summary of what happened; please provide a concise summary and be sure to include all pertinent information.

- When it happened
- Where the incident occurred
- Who was involved
- How were you affected
Step 4 - Submit the claim
Depending on the plan you purchased, and when you purchased it, you may be able to submit your claim online. Otherwise, you can mail or email the claim and all corresponding documents. Please see the Claim Forms page for more info on how to submit your claim.
