The first step to filing a claim is preparation.

Step 1 - Gather the necessary documents
Below are some required documents for common claim types. Documentation will vary depending on the type of claim you are filing. the claim form will have required documentation and if not provided, will slow down the claim filing process. If you traveled internationally, you may be asked to provide a copy of your passport showing the identification page and entry/exit stamps or provide a copy of your I-94.

Here are common claim types with a list of the documents listed that may be needed to file that type of claim.
MEDICAL CLAIM TRIP CANCELLATION or TRIP INTERRUPTION TRAVEL DELAY (QUARANTINE)
  • Medical documentation
  • Doctor and/or hospital reports
  • Detailed bills and/or forms for medical services received
  • Receipts for payments made for medical treatment
  • Original trip itinerary and invoice
  • New trip itinerary
  • Proof of trip payment
  • Proof showing the cancellation date, the amount forfeited, and the amount refunded
  • Proof of the event that cancelled or interrupted the trip
  • A statement from physician, government, airline carrier, or airport facility that documents your delay
  • Receipts for accommodations and meals

Step 2 -Find your plan information

Find the plan name, your ID number -which starts with a 9 and your order number - which starts with a 1 or a 2 and can be found on the confirmation of coverage or certificate emailed at the time of plan purchase. These numbers are required to file a claim. If you can't locate your purchase email - don't worry, just contact customer service using the chat button below and they can help you.

Step 3 - Complete the claim

You may file a claim online or pdf claim form can be mailed, faxed or emailed. Please provide a concise summary and be sure to include all pertinent information. Common questions include:

  • What happened
  • When it happened
  • Where it happened
  • Who was involved
  • How much you are claiming for

Other requested info includes banking information for payment directly to your account and a current email address for correspondence.

Step 4 - Submit the claim
Depending on the plan you purchased, and when you purchased it, you may be able to submit your claim online. Otherwise, you can mail or email the form and all documents. Don't worry if you forgot something. Once your claim has been reviewed, we will let you know if additional information is needed.

Important!

  • The claims team will reach out to you via email when the claim has been accepted and again when additional information is needed to process the claim.
  • Processing your claim will be delayed if you use the wrong claim form, mail the claim to the wrong address, or send in incomplete documentation pertaining to your claim.
  • Any flight cancellation or delay must be documented by the airline.
